Small gaps and unaligned track

I am finding small gaps and bits of broken track.
At first I thought it was bad joining on my part.
But these gaps are not appearing at construction joins.
They appear anywhere, even in the middle of bridges.
Trains make it over the gaps with a jolt and it
does not seem to affect the flow of electrification.
When laying track next to one of these gaps, the gap
will also appear replicated in the newly laid track.
These also appear as overlapping bits of broken rail,
One piece not aligned to the other.

It was noticing something strange in the movement of trains that
lead me to find these gaps.
